Resident Care Aide

Hours of Work:

  • 6:45 am – 3:15 pm
  • 2:45 pm – 11:15 pm
  • 10:45 pm – 7:15 am

Resident Care Aides are responsible for supervision of residents, personal care, and basic housekeeping tasks in accordance with the Fredonia Place Assisted Living philosophy. The Resident Care Aide is charged with the responsibility of ensuring the safety, welfare, dignity, confidentiality, and comfort of each resident. Assist residents in meeting their physical, emotional, spiritual, and medical needs to achieve and maintain their highest level of wellness.  She/he is in communication with the Director of Resident Care and/or shift LPN, at minimum, once per shift to provide a report (verbal and/or written) of each resident’s general welfare with special attention to any needs, issues, or changes in condition.

  • Experience in Point Click Care a plus but not required.
  • Experience with the geriatric community a plus.
  • Sit and walk intermittently throughout the day.

Med Tech

Hours of Work:

  • 6:45 am – 3:15 pm
  • 2:45 pm – 11:15 pm
  • 10:45 pm – 7:15 am

Med Tech’s are responsible for medication assistance, supervision of residents, personal care, and basic housekeeping tasks in accordance with the Fredonia Place Assisted Living philosophy. The Med Tech is charged with the responsibility of ensuring the safety, welfare, dignity, confidentiality, and comfort of each resident. Assist residents in meeting their physical, emotional, spiritual, and medical needs to achieve and maintain their highest level of wellness.  She/he is in communication with the Director of Resident Care and/or shift LPN, at minimum, once per shift to provide a report (verbal and/or written) of each resident’s general welfare with special attention to any needs, issues, or changes in condition.

LPN

Hours of Work:

  • 6:45 am – 3:15 pm
  • 2:45 pm – 11:15 pm
  • 10:45 pm – 7:15 am

Oversee and promote the residents’ health and wellness through administration of all residents’ medications, regular visits with each resident, resident monitoring, and ongoing communication with the resident care team.

The LPN uses his/her nursing knowledge to ensure the Resident’s care are adequately and appropriately assessed and met. Responsibilities include the cooperation, coordination, supervision, and enforcement of the DOH regulations, Fredonia Place policies and procedures. Oversight of the day-to-day operations of Memory Care to ensure compliance with company policy and regulatory requirements as well as the liaise between Residents, their families and/or Legal Representatives, and community-based service providers. Additional responsibilities include in-service education as well as appropriate documentation requirements. As one of the lead members of the Nursing team the LPN works directly with the Director of Resident Care to facilitate compliance of the Resident’s individual plan of care. In this role, the LPN provides primary resident supervision in all areas.

LPN Administrative Services:

  • Responsible for overall supervision of the building in the absence of management.
  • Must be familiar with and able to implement emergency procedures as outlined in the Fredonia Place Emergency and Disaster Plan.
  • Responds to all requests for information and all other activities as directed by the Administrator, Director of Resident Care, Director of Health & Wellness, and all other department heads.
  • Maintains on-going infection control surveillance, OSHA standards, universal precautions and basic sanitation and safety in the facility including but not limited to the Nursing Department.
  • Assures the accuracy of Resident records and documentation in adherence with company policies as well as regulations of the New York State Department of Health.
  • Assures the medication management system adheres to Federal, State, local Department of Health laws and regulations and company policies.
  • Follows the policies and procedures related to incident reporting as well as Department of Health Regulations and Fredonia Place polices.
  • Maintain active community and professional relationships to support positive promotion of Fredonia Place.
  • Maintain ongoing communication with the resident, resident’s family, and physician regarding the resident’s medical needs.

Server

Hours of work:

Flexible to the needs of the kitchen.

The Server is responsible for serving food and beverages in an efficient, professional manner according to Fredonia Place Assisted Living philosophy, policies, and procedures.

Duties and Responsibilities:

  • Complete all assigned opening/closing duties to prepare dining room, kitchen and service stations for efficient operation during meal periods.
  • Follow all Fredonia Place policies and procedures.
  • Serve residents and/or guests in a friendly, professional and timely manner as per training guidelines.
  • Maintain, clean, and organize service areas and stations such as the kitchen, dining room and service stations prior and during meal periods.
  • Assist team members as needed throughout shift.
  • Perform other related duties as assigned by any supervisor.

Cook

Hours of Work:

Flexible to the needs of the kitchen.

The Cook is responsible for making all food, which includes breakfast, lunch, dinner, desserts, snacks, etc. in an efficient, professional manner according to Fredonia Place Assisted Living philosophy, policies, procedures, Department of Health regulations and the New York State Sanitary Code. Assist residents in meeting their nutrition intake and goals to achieve their highest level of wellness. She/he is in communication with Dietary Director and/or On-duty Cook, at minimum, once per shift to provide a report (verbal and/or written) of each resident’s general welfare with special attention to any needs, issues, or changes in condition.
